Transitional Care and Care Management
Beth Lopez RN, BSN, CCM has been responsible for day to day operations of Transitional Care and Care Management programs. She designed a Transitional Care program to meet the contract goal of reducing 30 day readmissions of the Medicaid Managed Care population. Beth also served with the county Community Resource Connection’s (CRC) Transitional Care Committee and aided in the development of a pilot program to work collaboratively with the local hospitals to reduce readmissions of their high risk populations. She graduated from North Carolina A & T State University with a BS in Nursing. Additionally, she served as a nurse in the US Air Force and brings with her a wealth of clinical experiences throughout her career, including: ICU, dialysis, triage and managed care. Beth obtained her certification in Case Management from The Commission for Case Manager Certification in 2005.
